Mercy Care’s social model of informal elder support services provides opportunities for greater stability and continuity in older adults’ care. Availability of community support services have long-term health consequences and are part of the new health care model.
Mercy Care recognizes that according to the New York State Office for the Aging, by 2040 in the Adirondack North Country region, which includes the counties of Clinton, Essex, Franklin, Hamilton, Jefferson, Lewis, and St. Lawrence, the only population growth projected is in the category of people over the age of 60. Social isolation and loneliness is recognized as a social determinant of health. The need for Mercy Care’s services is growing rapidly.
Mercy Care offers its services in cooperation with families, community organizations, community leaders, health care providers, agencies serving elders, and churches and congregations of all faiths.
